INSTRUCTIONS
On the Red side, the person must enter his name, date,
time, and tools used for the job.
GREEN SIDE CHECKLIST FOR ENTERING THE DERRICK
VIA ELEVATOR OR WORKBASKET
1) Notify the Driller.
2) Sign in the Derrick Log Book with T-Card #. Print name,
date and time up on the Red side of the card.
3) List all tools to be used for the job in the Log Book and
on the Red side of the T-Card.
4) The Driller must visually confirm all tools that are to be
used.
5) Ensure that all tools are tethered or secured.
6) Is a JSA required?
7) Is a Safety Harness required?
CHECKLIST FOR EXITING THE DERRICK
1) Notify the Driller.
2) Ensure that all tools are accounted for.
3) The Driller must visually confirm that all tools have
been returned.
4) Remove name, date. time and tools from Red side of
T-Card. (Grease pencil or white board marker)
5) Sign out log book and close out PTW if job is finished.
